A support form for end users to ask for help has been requested. A number of gen'l requirements drive this:
1) Need to capture which gateway/dataset the support query may be related to.
2) Need end user identiry (openid, gw)
3) Need end user country for US export ctrl.
4) Need to direct end user to support FAQ and other "first tier" help documents.
