      CADIS references to OpenID should be removed.

      CADIS doesn't use openID, but these are referenced in several places, including:
        MyProxyLogon link on download pages
        Forgot Password Retrieval page (may be configuration issue)
        New Account confirmation email (may be config/template)
        Third page of new account reg workflow.
