Problem: ESG Account "List Current Memberships"->Remove confirmation dialog is not displayed correctly. The dialog functions with "close", "yes" and "no" links. However, it is very difficult to see as the boarder and background are not shown.

Steps to repro:

1) Login to esg.prototype
2) Click Account nav menu item.
3) Click "List Current Memberships" Link.
4) Select group in list
5) Click "Remove" button

Confirmation dialog is shown, though difficult to see.

Acceptance Criteria: Make it work similar to the previous functionality using Bootstrap.
